Process Hazard Analysis (PHA)
- Review Plant Safety Sign-In Procedures & Training
- Sit down with plant manager, plant engineer, and plant process engineers
- Tour plant
- Meet with safety engineer
- Review if PHAs have been performed and date of last certification
- Review PHAs to see if they were rigorous
- Review status of PHAs action items, i.e. have they been complete
- Review PHA criteria, such as P&IDs, CSDs and sequence of operations
- Review PSSR paperwork
- After review of PHA documentation, re-tour the facility
- Review plant's MOCs
