Process Hazard Analysis (PHA)
  1. Review Plant Safety Sign-In Procedures & Training
  2. Sit down with plant manager, plant engineer, and plant process engineers
  3. Tour plant
  4. Meet with safety engineer
  5. Review if PHAs have been performed and date of last certification
  6. Review PHAs to see if they were rigorous
  7. Review status of PHAs action items, i.e. have they been complete
  8. Review PHA criteria, such as P&IDs, CSDs and sequence of operations
  9. Review PSSR paperwork
  10. After review of PHA documentation, re-tour the facility
  11. Review plant's MOCs
